Your request is sent only when you select “Send booking request” on the review screen. Your details and attachments are used to review and arrange the inspection. Dates and scope remain subject to confirmation. Unsaved entries are lost when you leave this page.

You can use either the online preparation form or a downloaded form. The original templates include English fields and a bilingual declaration.

The downloadable forms are the original templates and still show a signature line and references to service terms. For this booking process, a signature is not required, and service terms will be provided separately for agreement.

Parts eligibility is reviewed for the destination. The original form contains a restricted used-parts list; it is not applied here as a universal automatic rejection rule. Provide the complete packing list for review.

  • To the best of my knowledge and belief, the products listed in this request are not subject to product recalls and are neither substandard nor counterfeit.
  • I declare that the listed products meet the standards and other applicable requirements for their export.
  • I agree that Garage Ono may share information, results and documents relating to this inspection with the relevant authorities where requested to verify product conformity.
  • At the time this request is submitted, the shipment remains in the country of supply and is available for inspection.

Acceptance is recorded by the required checkbox below; no separate signature or signed declaration PDF is required for this booking process. Service terms are being prepared and are not accepted through this checkbox. Any applicable service terms will be provided separately for agreement before the service proceeds.
